Job Description:
A company is looking for a Registered Nurse Care Coach to work
remotely and support patients in Medicare's Chronic Care Management
Program.
Key Responsibilities:
Utilize care management software to engage with Medicare patients
with chronic conditions through monthly calls
Build rapport with patients to coach them on self-management
strategies and improve health outcomes
Implement and enhance the Plan of Care by updating patient
information and connecting them with community resources
Required Qualifications:
Current, unrestricted New York nursing license
Proficiency with electronic health records (EHRs) and web-based
applications
Minimum of 3 years' experience as a Registered Nurse
Self-directed and able to work independently while meeting
performance metrics
Good with technology and eager to learn new software
